  • Document Home Medications at Admission. The first step upon admission is to document all of the medications the patient is taking at home in the Home Medications module.
  • Reconcile Inpatient Medications with Home Medications at Any Time. Medication reconciliation can take place at any time during the patient’s stay and is especially critical when the patient moves from one care setting to another, such as into or out of intensive care. Clinicians can analyze medications easily using a grid - sorted by therapeutic class - of the patient’s home medications next to the list of Centricity Enterprise inpatient medication orders, and the list of planned discharge medications.
  • Document Medication Reconciliation. Physicians can document that they have reviewed and reconciled the current active medication orders.
  • Reconcile Medications at Discharge. When preparing the patient for discharge, the grid makes it easy to review home, inpatient, and discharge medications as well as add new prescriptions. The clinician can identify duplicates and make appropriate adjustments quickly and easily. What's more, potential oversights are avoided, because the discharge nurse is alerted to new medications added at the last minute.  
  • Document Instructions for Each Patient. The Discharge Instructions form is organized into the following sections to create a complete set of instructions: Diet, Activity, Active Inpatient Medication Orders, Active Outpatient Prescriptions, Additional Prescriptions, Non-Medication Prescriptions, Discharge Instructions, Follow Up Instructions, and Patient Questions. Based on the needs of your organization, Discharge Instructions can include specific core measures, such as smoking cessation information.
  • Create Flexible Patient Education Templates.   Organizations can create templates to provide consistent patient education information. Templates take advantage of quick-entry features, such as check boxes, pick lists, free text, etc. The result is a fast and efficient way for clinicians to create and customize patient education materials.
  • Add Third-Party Patient Education Leaflets. Organizations also have the option to add patient education leaflets from content providers, such as Lexi-Comp, Inc. Clinicians can select leaflets directly from an index of available leaflets and the link is added to the form. Once the instructions are finalized, the specified leaflets print out along with other discharge instructions.
  • Give Easy-to-Understand Instructions to the Patient. When the document is finalized, the application formats the document for easy reading. For example, medication frequency and route are converted to more easily understood terminology for the patient. Prescriptions entered on the form can print out according to your organization's requirements. Lastly, patient education leaflets will print along with the patient instructions.
  • Access Notes from the Patient’s Record. Medication Reconciliation/Discharge Instructions uses the standard Centricity Enterprise document management interface to make completed notes available through Centricity Enterprise as a permanent part of the patient’s health record.
